To receive a refund of your remaining balance, please submit the following information so we can locate your account. Depending on the information we hold on you we may request a bank statement to verify the bank details where the funds need to be sent.

Your bank account details and personal information submitted in this form (“Your Personal Data”) will be used for the purposes of locating the outstanding balance on your Payfriendz prepaid Mastercard and refunding you the remaining funds.
